Ordering Guidance

This test does not include a pathology consultation. If a pathology consultation is requested, PATHC / Pathology Consultation will be ordered and performed at an additional charge.

Necessary Information

Pathology report (final or preliminary) must accompany specimen in order for testing to be performed. At minimum, it should contain the following information:

1. Patient name

2. Block number-must be on all blocks, slides, and paperwork (can be handwritten on the paperwork)

3. Tissue collection date

4. Source of the tissue

Specimen Required

Submit only 1 of the following specimens:

Preferred:

Specimen Type: Tissue slides

Slides: 1 Hematoxylin and eosin stained and 6 to 10 unstained

Collection Instructions:

1. Submit 1 slide stained with hematoxylin and eosin.

2. If the formalin-fixed paraffin-embedded (FFPE) biopsy specimen is a core needle biopsy or fine needle aspirate, submit 10 unstained slides.

3. If the FFPE biopsy specimen is a resection, submit 6 unstained slides.

4. Submitted slides should be 4- to 5-micron thick sections placed on positively charged slides.

Additional Information: Paraffin embedded specimens can be from any anatomic location (skin, soft tissue, lymph node, etc).

Specimen Type: Tissue block

Collection Instructions: Submit an FFPE tumor tissue block.

Specimen Minimum Volume

Minimum 60% tumor with or without macrodissection. Minimum required unstained tissue section input: 0.12 mm(3); Slides: See Specimen Required
